The code stamped on the 15 inch channel I need to replace is 14 3 GS CALDWELL.  Does 14 refer to sash weight?

I'm not sure what you have, unfortunately. We have dozens of different balances with that stamp. It's not a product code or anything, it just indicates the strength of the spring, and then only abstractly. The "14" for example is not directly representative of the actual weight of the sash. 

Our service requires clear and unobstructed photos of the hardware in question at minimum. In order to figure out what Series this is, I need you to remove an undamaged version of this balance from the window and show me clear pictures of the top and bottom nylon fittings. I will also need to know the dimensions of these fittings.

Top fitting - 7/8" wide, 1 1/4" high, 5/8" deep
Bottom fitting  - 7/8" w, 1 1/16" h, 1" d

Thanks! If your bottom fitting is 1" deep, then my recommendation is the Series 380. This has a 19-005 top and 15-005 bottom, so as long as those match up then you should be good to go.
