Identifying name is MEDALIST, B, 3/4 front and 1 3/8 rear. 2 diagonal slots on right side and 2 screw holes on left side.

Thanks for posting. Medalist brand parts are unfortunately not made anymore. We won't be able to supply you with this exact part. The only parts we sell that may work are our 23-009 and 23-010, but they're still very different and it is not a guarantee.
