I couldn't find any serial numbers or any kind of tags at all. This door is an older one, and parts has been replaced on it before. If your outside, looking at the door, the pivots are on the right side, and the door opens out.

I believe you have a Kawneer brand commercial door. Take a look at our 79-123 pivos, which have been used on Kawneer doors.
That's if you need a replacement. Looking at your bottom pivot, it almost looks like all you need is some adjustment to raise the door. The pivot has a threshold pin which can be raised using a 3/4" open end wrench. What's also nice about your bottom door pivot is that if you remove the cap screw you will expose an Allen wrench which you can also use to increase the height of the door. I would try this first before you purchase any replacements, because then you wouldn't have to remove the door from the frame.
