Counter balance failed on one side - 48X84 non tilt casement

Non Tilt Casement side counterbalance failed. Blew out top plastic cap and came flying out of slot!! Believe bottom anchor hook failed along with other issues, but otherwise window seems fine. Metal channel part of counter bar measures 28" exactly. Have additional pics if necessary but can't add to this post due to your 5 pic limit. The top cover cap also no good but not sure if that important to the full functionality.

Perhaps these additional photos will make it thru!! Also, the marking on side of metal channel was 27-6.
