Hi, I weighed the sash to 28.8 lb. The marking on it says 2790 and is a 1/2 inch width. Which part matches that?

This looks like an S395-28 Channel Balance; just look over all its specifications to confirm it seems right. For a window sash that weighs 28.8 pounds, use a stamp option of 2750, which will support a window sash in the weight range of 26 through 30 pounds.
