It appears that you are in need of a new pivot shoe. To further review, are you able to provide written out dimensions of the width and thickness of the shoe? We will also need to see a picture of the front, where we can see the size of the cam that engages with the pivot bar on the window sash. Thank you! 

I do not see any pictures of a cover. Are you able to show us pictures?
