Hi Alice,
Thanks for reaching out! This operator cover reminds me of the 39-323 Truth Hardware Contour Cover. The handle can likely be replaced with either the 39-114L or 39-205 (folding) crank handles.
If the worm gear has become stripped from the base, you'll need to replace the entire operator to repair the window.
