The casement window lock you have appears to be an exact match for the 38-092 Multi-Point Sash Lock. The left handed version is the 38-091. Be sure to compare the dimensions and design of the lock to confirm.

I believe you may be able to replace your tie bar with the 38-918 42.9" Tie Bar 3, Cone Roller. You'll want to double check that your roller does have a cone shape.

The closest replacement match we have for your tie bar is the 39-448 Tie Bar Guide. It does have four installation holes but I am confident this should work for your repairs.
