Based on my research, it appears to be a 27 inch BSI 2630 60G Extended Balance, but the top end has two "shelves" and doesn't seem to match any I've been able to find online. Also, the hook may be discontinued.
Could you please tell me if these are still available, and if not, the proper substitute?

Thanks for posting! Your window balances are custom, Extended-Travel Channel Balances. The length of the channel should be 26-7/8".
To confirm pricing and availability for them, I'll need to know the number of balances you're looking to purchase. The quantity minimum is (2) balances.
