It looks like your balance may use the 19-005 top and 15-029 bottom fittings. The 15-029 is not available with any of our stock balances but we can supply it through a custom order as long as the 7/8" deep measurement below matches your shoe. We'll also need you to confirm the exact weight of your window sash, as well as the number of replacements that you need with the minimum being two. Thank you!

3630: 18 to 23 lbs
3640: 23 to 30 lbs
