is this a s 395 27 balancer with a 19-005 top and a 15001 bottom and option 2630

Hello! I agree that the S395-27 looks similar. As long as the dimensions we show for the 19-005 top and 15-001 bottom match yours then you should be good to go!

Yes, it does correspond to your stamping of "26 3" which holds a sash spring strength of 13-19 pounds.
