I believe I found it at DK Hardware:

Thanks for your post! We do not have this exact pivot shoe but the 15-211 should work in its place. The nylon cam on our shoe differs from yours, so may also need to replace the pivot bar. I'm happy to recommend a replacement, I'll just need to see a photo of the original.
