Hello, Jon. This looks like an old Certainteed brand coil balance assembly. We sell the 15-192, which should replace your old pivot shoe, but I believe yours is an older variation; the major difference being that our shoe has front "aligners" that tend to get in the way in older applications. They're those bumps you see on the face.
With that in mind, this shoe should work for you if you're willing to shave those aligners down. You can use a utility knife, or anything else you're comfortable with. Just make sure to wear safety gloves if you go that route.
It also appears that the shoe is the only damaged component. Assuming your spring is in good condition then it can be reused without issue.
