Your handle set reminds me of our Legacy Keyed Handle Sets; they are the 82-086 (available in white) and the 82-087 (available in brass). To confirm that this style is the right replacement, compare the distance between the installation holes as shown below.

You'll also want to be sure to double check that your latch is in an offset position as well.
