Thanks for your post! The door handle you have can be replaced with the 82-030 Sliding Patio Door Handle Set. If you are interested in a keyed version, take a look at the 82-030A Centered Keyed Handle Set. The mortise lock inside the door appears to be the 82-227 Mortise Lock, which is sold separately from the set.

Take a look over the dimensions for each part to confirm that these are what you need for your repairs.
