Your mortise lock resembles the 82-226, which is commonly used with handle sets with 3-15/16" hole spacing such as the 82-030. Unfortunately, we do not carry a handle set like yours with exactly 4" hole spacing but 3-15/16" is very common. I would recommend double-checking your measurement to confirm if our replacement could work for your door.
