This is from a drawer by Fine Furniture Design.  There are no markings on the glide.

I am afraid we don't have this drawer guide; it is no longer available. You may be able to replace the entire track with something like our 32-053, but I can't guarantee it. I apologize we can't be of more help, and best of luck with the repair.
