Drawer slide rear hardware

House built 1989, original cabinets. Can't seem to figure out what this part is of whether replacements even exist withiut redoing all hardware. Any help would be MUCH appreciated!

We can definitely help you with this project, though not with an exact match. Your old socket isn't made anymore. These days, cabinet manufacturers like using drawer sockets that screw to the back of the cabinet.
In your case, I'd look at our 32-018 to start out. Carefully look over our dimensions, especially the size of the opening that accepts the track. That will tell you for sure if it will work.
You will have to drill new holes to the back of the cabinet. I hope that's okay.
