Dyad Window Operator, hardware attached to the handle pops out

If part of the operator is still attached to the handle and popping out from the gear it sound like the operator is broken. If this is the case the operator cannot be repaired only replaced.

The hardware attached to the handle unscrews and pops out when trying to close the window. Any advice what the problem might be?
