Hello there. This looks like it might be either the 39-031 or 39-156 Dual-Arm Operators, depending on the length of the link arm. Please compare the dimensions below with your old hardware and see which model matches best. In either case, for a new cover and handle, see the 39-107 and 39-114L, sold separately.
