Hi Steve. This looks like its our 39-031 and 39-032 Casement Operators. Just compare all the dimensions we list in the pictures on the shop pages to ensure this type matches what you need. For new covers and handles, see the 39-107 and 39-114L. The video below shows how to remove and replace this operator.
