19 5/8" channel length, stamp: 19 3 RL 8.S.I., mfr: Milgard, only the hook is broken. Can I purchase just the hook? Otherwise, I need a quote for 2 balances so tension is same left and right.

You can buy the hook, yes! Yours looks like a stretched-out version of our 62-108.

Keep in mind that this kind of damage is usually a result of the balance spring failing; when the spring gets weaker, it struggles more and more to support the weight of the sash, which puts more strain on the hook. It would stretch out much like yours has. 

We can supply the balance, if you'd prefer, but it has to be a custom order. Just let me know if you wanted to go in that direction instead of just getting the 62-108 and I would be happy to set you up.
