Hello,

With this combination of attachments, you could have either the S380-27 or S385-27 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

You'll also need 2630 spring strength options for your replacement balances. Thank you and let us know if you have any other questions or concerns.

How do I get the 2630 spring strength options? Is this another part?

To clarify, my stamp says 26C USB. What weight do I choose? The third one down for C???

2630 is one of the strength options listed on the S380-27 balance product page. Your 26C stamp would correlate to 2630 for our balances based on C being the third letter in the alphabet, which will support a window sash between 13-19 lbs.

I have ordered the correct balances based on your recommendations. Your Order # 2177401
I also ordered 18-008 - 5/8" Top Sash Guide. How do these attach? Do I need to remove the old rivets and replace them? Do you sell replacement rivets or bolts?

You shouldn't need rivets unless you are replacing the balance fittings but we do have videos on replacing your non-tilt channel balances and top sash guides shown below. Take a look and let us know if you have any other questions.
