manufacturer is unknown. Window is wood with wood frame. 38.5W x 46.5H weight is 46lbs Chanel depth is 3/4 and width is 5/8 metal channel without end caps measures 44" and is stamped with 4370. inside the window frame, there no retention clips as shown in vinyl windows, there is a 3/4 pilot hole with a string and metal end that look to be the only attachment point for the balancer, and it just kind of lives loose in the channel. I have additional photos, but was limited to 5

Will need 2 as both sides have fallen apart.

The replacements will be identical to your old hardware so it should indeed be compatible with the top sash guide shown here.
