Need Qty (2) balances, plus the plastic mating parts that are mounted on the sash (see last attached photo).

Thank you for reaching out to us. This doesn't look like an extended travel channel balance. What you have here is a match for the S351-34 or S390-34, depending on the depth of the bottom shoe. You'll have to measure as shown below to determine the correct Series.
Have you ever had any issues opening or closing the window? I ask because the 3340 stamp on your balance is calibrated for a sash weighing 23 to 28 pounds. This stamp wouldn't be strong enough for the window sash if it weighs 34.3 pounds.
You may be better off with the 3360 stamp option; a pair of balances with this stamp will support a sash weighing 32 to 35 pounds.
As for the top sash guide, take a look at the specifications for the 18-008H and see if they match what you need for this repair.
