This bracket is 2-15/16" wide and 1-3/8" tall on the side that mounts to the slide. On the cabinet mounting side it is 3/4" wide and 1-11/16" tall. Spacer distance is 1-13/16", mounting surface to shelf side. No markings on the part, but the slide rail has LC and 36-05 marked on it.
The mount has been glued to attempt a repair. It lasted several months, but just failed again. The opposite side (right) part is also needed.

We don't carry the drawer spacer you show in your photos, but we do have a few that are similar. Take a look at the two tallest drawer spacers we carry: the 32-334, which is 1-1/4", and the 32-350, which is 1-3/4". See if you can use either of these. Worst comes to worst you could always build up with washers to get the distance you need for your drawer.
