A home owner says:
Both Guide pieces are worn so they came off the guide bar when closing the crank. Is it possible to replace the guides alone? If so, not sure how to put onto guide bar since each end of the bar is flat at each end where it screws to window frame. Or, do I have to replace the bar with guides already attached? Thanks

Behind that center bracket you show in one of your photos is where the two bar guides meet up in the middle. I recommend that you remove the two screws holding the bracket to the window so you can install the new shoes. That should do it.
