HS+D MW wooden sliding door hardware

Bought a house shell that has been standing for 11 years. Four wooden sliding doors had been installed with the internal hardware already installed as well. The doors where made either by MW or Ply-Gem. I have the hardware (photo uploaded) for two of the four doors. This hardware was made by HS+D the name being Alberta Sliding Door Handle Set. The PO NUM on the box is 608711-000 OP and a FB SO NUM of S-3479.

Would appreciate any information you may be able to provide to purchase two additional sets.
