There is nowhere on the door, or anywhere that has a name or part number other than the numbers in the one picture

Unfortunately, we do not carry a replacement handle set that would be compatible with the multi-point mortise lock shown in your photos. I'm sorry that we won't be able to assist with this repair but please let us know if you have any other hardware questions. Thanks for posting.
