Hanging drawer rear mount parts HELP!

A quick learner from Little Rock, AR says:
January 25, 2020
Hanging bracket is 4 7/8ths"L with about a 5/16ths hole. The rail is 21 1/2" long. I front know what the plastic off white piece is that connects the two. It is 2 3/4 total long. Can you help with finding replacements??? Please? Thank you!
