Thanks for your post. We do not have this exact top sash guide but our 18-046 might work on your sash. The width appears to be very close and the top two holes seem to line up with those on your guides.

Be sure to compare the dimensions below to determine if this will work for your repairs. You'll especially want to compare the depth of the guide; the length is not critical.
