Hardware - Lift hook - Sash guide for a spiral balance

A handy person from Scenic Gainesville FL (the Swamp) says:

Hi.... These are on a 3/8 tube balance, 28" long with a 30" spiral rod. The sash is no more than 20# which might correspond to a "red" type balance. I need either just these parts or a complete unit. Thanks Much, Bernie
