Thank you for reaching out to us. Your old balance reminds me of the S770-34. Our balance installs at the top of the window track with a screw, however, yours appears to use an S-clip. We can still help, but this must be a custom order due to that difference. Can you confirm the number of balances you're looking to order?
Regarding the pivot shoe, the 15-135 looks like a match, but see if the thickness and width align with yours. If you'd like us to ID the pivot bar, we'll have to see it removed from the window sash.
