Your operator looks like a match for our 39-239B. Check it out and tell me if you agree. We also sell the 39-536 pivot slides and 39-375 cover separately.

Your hinge isn't made anymore, but I believe you can use our 36-048 as a substitute. Just make sure you replace both hinges on the sash at the same time, you don't want to mix and match old with new.
