I've look everywhere and can't find anything similar to it. I belive is a 3/8 x 27

Hello there. This reminds me of this old wood window balance seen here. Double-check that diameter. If its 5/8" I think you'd need the S500-27 Spiral Balance, with a 17-012 Bracket Shoe. Take a look and see what you think.
