A home owner from CA says:

Series : 810S

Type: Single Hung

Label on Channel Balance: 28 4 EW CMC 4 (I don't know how to read this. What does this say?)

Mental Length: 29"

Width: 5/8 or 1/2"

Depth: 9/19"

Top Attachment: 19-005 (with Wings. I am guessing here!)

Bottom Attachment: 15-004 (13/32" depth. I am guessing here!)

What is the Option? (It won't let me proceed without selecting an option. And I don't know the Sash weight. Is it 2840? 

Could you please verify what I need against the pictures?

A stamp on the balance of "28 4" typically indicates a 2840 stamp option. As for your balance Series, if you have a 19-005 top and 15-004 shoe, you will need a Series 385. The 29" version is our S385-29.
