Help finding top sash guide

Please help me find this part. I need at least 6 of them.

When looking at window side-on across top dim 7/8" as width. Main width 5/8".

Height 1 5/8", from top to screw 1 3/8". From top to channel balance catch 11/16"

Upper Balance catch depth 5/16". Lower depth 1/8".

They are used on a nontilt sash window. The channel balances used are 380 series
