Help identify window channel balance

Hi,
I want to replace the window channel balance. It is 1/2" wide and metal length is 28". I need help figuring out the exact design of the plastic ends to order.

Hi there, I believe you have an S350-28 channel balance. This balance comes with the 19-002 top and 15-004 attachments riveted on. By sure your shoe depth is 1-3/32" like the 15-004. Your spring weight option will be 2740 to correspond with your 27 4 stamp. Good luck with the repair!
