Help in identifying "shoe"

Question

The attached pictures are of a "string" failed Certainteed counterbalance, circa 2002, off of a vinyl single-hung window.

I'm pretty sure the top plastic portion is a 19-005 but not certain about the shoe. It appears it could be a 15-001 or 3,4 or 5?? Can you help?

The metal portion of the spring assembly is 26" long. The marking on the top of the spring/counterbalance are 25 5 FU CMC 2, as shown in Spring 2.jpg

Hello, Nancy. I believe the shoe is either our 15-004 or 15-005, considering the fact that it has wings (the 15-001 and 15-002 do not have wings). However, you will need to compare our measurements for these shoes to your own to confirm which is better. That's the only way to know for sure. If it's the 15-005, consider our Series 380 channel balance. If it's the 15-004, consider our Series 385 instead.
