Help with 5/8" Top Sash Guides

top slotted hole at 3/16" below top of part
bottom (centered) hole down approx. 3/4" down from top of part
part length overall 1 1/2"
sorry I have no pic of part off but back of part is flat
Do not know the window brand
I intend to replace both sides so they match

The top hole of your top sash guide is off center. The closest match I could find is 18-008H. Look over the measurements and see if this will work for you.
