Thanks It's pretty close (32-147) but the long side should have one hole in the center.

Unfortunately, we no longer carry the hinge assembly shown in your photos. The 32-147 would be our best replacement options at this time but adaptations such as making new installation holes would be necessary. I'm sorry that we won't be able to offer a closer match but please let us know if you have any other hardware questions. Thanks for posting.
