Hello,

We have a couple of hinges similar to this style. You can start with our 32-340, which I think might be the closest option we have to your hinge. Look it over and see if you agree. 

It's not an exact match, so you may have to redrill the installation holes but it could also work as-is. You'll have to see how it fits to determine if any modifications are needed. Thank you!
