2. Aluminum Handles/ Wood insert, interior handle only. Screwholes @ 4" C-C. Latch Lever @ dead center of screw holes..
3. Latch Mounting holes @ 3 3/4" C-C. Latch Slot @ 90* (horizontal to vertical). Latch Body width @ 1/2".
4. NO identifying numbers, marks, etc, on Visible surfaces - interior or exterior.
5. Very heavy glass.. Custom built home, completed in '68. (Quality, Original.)
6. HAVE both replacement (Floor rail) Rollers in hand.. to be installed.
7. *Found, past research* Listing of this Door Handle. "Discontinued" Can't remember Make.
8. CAN get Pics - depending upon response of, above, Dimensional information.

I'm afraid I don't know what handle set you have based on the provided information. It's unfortunately not enough. We will need photos of the hardware in question in order to proceed. A photo of the corresponding mortise lock would also be very helpful.
I would also ask you to triple check the center-to-center hole spacing on the handle. 4" is not a size I've ever encountered, but 3-15/16" is very, very common.

1. C-C Triple Check: Yes Sir, the 'off size' issue struck me for this Commodity Item! It IS the 3-15/16".
2. Further deep research reveals far Fewer overall Problems to the original Issue.. The Mortise Lock, itself, IS Common. "Off the rack" hardware store item.. Solved.
3. The cast aluminum Interior Handle IS "Discontinued" - by being out of Style, and rather Indestructible! The *Handle Issue" Should be a simple Fix: It's all in the Exterior cast 'pot metal'/ Common/ handle that's *stripped out* it's #6-32 threads.
This common Handle is stamped "500-14A", and should be an Easy 'cross-reference' online..
4. Lastly, it becomes painfully obvious that most Issues with Sliding Glass Patio Door *Handles* are a direct result of Poor Maintenance and/or Attention To the Lower Rollers/ Track of the Door, itself. Period.
This Causes the "rough service" to the Door Handles resulting in 'premature failure."
