We can help with this hardware, but note that we don't sell this balance as a full assembly. We only sell its individual components.

Start by looking at our 15-111 pivot shoe. This appears to match your original but let me know if you agree. Next, remove the original spring from the window and tell me how thick it is. That will allow me to determine what spring and cover to recommend.

It also looks like you need a tilt latch. See our 26-243 and 26-244 to determine if these will fit in your window.
