Hello, I can't say with certainty what pivot shoe you need while it's still installed but I suspect that either the 15-178 or 15-188 would work for you depending on which thickness matches your original. Based on the information you've provided about your balance, the S900-27 looks like a strong contender. Keep in mind that your balance appears to have a yellow tip but the sash weight you provided (21.4) would fall into the weight range for a red tip.
