I believe our 18-171 top sash guide will be a match for yours. Your exact type isn't made anymore, but many customers have reported that the 18-171 can work if you cut the top off.
For the balance, start with our Series 380 channel balance. This comes with a 19-005 top and 15-005 bottom, so as long as those match you should be okay.
