I would like to know where to go for spare parts here ...

This stamp tells us the strength of the balance, but not its identity. To know this, please remove it from the window and show us the nylon fittings on either end. Their design will tell us what you need.

Is there an identical window in the house that is undamaged? If so, please remove the balances from that window and show us pictures of the fittings on either end. It also looks like the Top Sash Guides are damaged. The sash guides are the nylon pieces in the corners of the sash that keep the balances in place. Remove an undamaged version and show us pictures of it from the front and side. See the videos below.
