Hello! I am unsure which option to choose. I am 99% certain that I need this part, but I am unsure as to the last option.

Thanks for posting. Good deduction. I agree this looks like an S390-35 Channel Balance. Just make sure that bottom nylon shoe has a depth of 1-1/8", as shown below. If it's 1" instead, you're going to actually want the S351-35.

In either case, for the stamp option, choose 3440. This accords with your old balance's stamp of 34-4. Let me know if you have further questions.
