Hi there!

The operator in your photos appears to be an older version of the 39-091 and 39-092. However, the link arms were discontinued and I'm afraid ROTO didn't offer a substitute. 

As for the hinge, take a look at the 36-004-10. This should work for you if you replace the top and bottom hinges at the same time. Review the specifications and see what you think.
