Identify Top Sash Guide

A home owner from Jacksonville, FL says:
Please help me identify a replacement for old top sash guides.
Dimensions, according to my measurements are:
Width at top: 15/16"; Depth at top: 11/16" (possibly 21/32)
Width at bottom: 11/16" (possibly 21/32)
Height (measured below top lip) 1-1/2" (possibly 1-17/32 to top of lip)
Drop to center of hole: 7/8" (from below lip)

The old guides are currently installed in single-hung, non-tilt, aluminum windows of two sizes. Most bottom sashes weigh approximately 30 lbs. Others weigh approximately 20 lbs. All are using S380-23 channel balances, option 2240, which appear to be a mismatch for the 30 lb. sashes. I intend to replace them all with appropriate balances.
